  • Yes! For patients with Medicare, Federal law protects the patients right to choose their preferred health care providers. Some commercial insurers will require members to choose from a list of contracted home health care companies. If your doctor or case manager is recommending home health care, you can request Reliant Home Health Care.

  • Depending on eligibility, Medicare may cover the entire cost of your home health care when your doctor orders and certifies the need for services. Private commercial insurances may also cover all, or a portion of your home health care depending on the plan benefits you are enrolled in. Reliant Home Health Care will confirm your benefits and communicate any out of pocket cost prior to initiating home health care services.

  • The frequency and duration of home health care services provided is tailored to each individual patients needs. Once our initial assessment and comprehensive evaluation is completed, we will work with your referring doctor and insurance carrier to approve a plan of care that ensures each patient receives the care and services they need.

  • Once Reliant Home Health Care has received your referral and has verified your eligibility, our office will contact the patient to schedule a clinician within 24-48 hours of hospital discharge or referral receipt from your doctor’s office.

  • Most of the time, patients need to be considered “confined to the home” or “homebound” to receive home health care services. Patients are considered “confined to the home” or “homebound” if they meet these two criteria:

    Patients either need supportive devices such as crutches, canes, wheelchairs, and walkers; special transportation; or help from someone else in order to leave their home because of illness or injury OR have a condition that makes leaving the home medically inadvisable.

    “There must exist a normal inability to leave home; and leaving home must require a considerable and taxing effort.”
